Grief and Bereavement Counselor- Part time Falls Church, Va.

The Grief and bereavement counselor provides care to families, children, significant others, and the community at large who are dealing with issues of grief and loss.

Requiresments:

* Masters in Social Work, Pastoral Counseling or Counseling.

* Must be licensed eligible (LCSW, LPC).

* Two years experience in health care, preferably in hospice or home care.

* Extensive knowledge of death/dying, grief loss and survivorship issues.

* Clinical experience with grief treatment in counseling role.

 


Nurse Educators (RN) Part time                                                                             WP

Reports to Director, Staff Excellence and Performance

Clinical Educator (Nursing) is responsible for the overall clinical education program for nursing staff.

Access Specialist- Full time- Falls Church, Va.            WP

As a member of the Referral Center Team, the Access Specialist is the initial clinical point of contact for patients; families; physicians; discharge planners and other individuals seeking information on access to care. The Access Specialist is responsible for: managing complex referrals, including patients with urgent symptom management needs and high tech equipment needs; all hospital referrals; transferring information to the Registration Coordinator; coordinating the insurance verification; following through to assure the delivery of quality care to all referral sources.

Requirements:

* LPN or RN with Current Commonwealth of Virginia Professional Nursing, District of Columbia and Maryland license.

* At least 1-2 years in clinical setting/medical office.

* Hospice experience desirable.

* Excellent customer service skills.

* Ability to multi-task and attention to detail.

 


Volunteer Coordinator                WP

Part time- Manassas, Va

Under the direction of the Director of Community and Family Services, the Volunteer Services Coordinator recruits, interviews, and coordinates training, daily activities, continuing education requirements, and performance evaluations for volunteers providing patient/family care, bereavement support, administrative assistance and other volunteer services in support of the mission of Capital Hospice.

Requirements:

* BA/BS or equivalent in human services, education or management required.

* Two years responsible administrative experience including program coordination or supervisory experience required.

* Hospice and or volunteer experience strongly preferred.

* Excellent organizational skills.

* Effective interpersonal relationship skills.

* Excellent verbal and written communication skills.

 


Professional Education and Practice Improvement Coordinator         WP

Full time

Reports to VP Medical Services Cameron Muir, MD

The Inpatient Clinical Educator (Nursing Practice Improvement Coordinator) is responsible for monitoring, evaluation and improvement activities for nurses at Capital Hospice inpatient unit.  The inpatient clinical educator will be responsible for supervising the hospice inpatient unit staff during the weekdays.  The inpatient clinical educator will be responsible for reaching, organizing and presenting all of the educational modules for nursing and advance practice nursing fellowship programs and staff education for all disciplines. 40% teaching/professional education, development and leadership; 40% clinical site coordinator for external learners; 5% clinical role; 5% clinical education and staff development; 10% quality patient care
