Capital Hospice has developed an electronic form (Excel) for submission of claims. After clicking on this link save the form to your computer. Once completed, it can be emailed to us at accountspayable@capitalhospice.org and payment will be processed once we have received all supporting documentation. You may use that email address for any questions regarding billing.
