Capital Hospice works with numerous health care providers and facilities throughout the national capital region. We provide our partners with tools to enable the best possible customer service for our sources of referrals.
Capital Hospice has developed an electronic form (Excel) for submission of claims. After clicking on this link save the form to your computer. Once completed, it can be emailed to us at accountspayable@capitalhospice.org and payment will be processed once we have received all supporting documentation. You may use that email address for any questions regarding billing.

Billing under Medicare guidelines can be confusing. We've prepared a Medicare Billing Guidelines booklet intended to assist in the efficient procurement of payment for physician services rendered to patients who are also receiving care from Capital Hospice. This information applies only to patients covered by Medicare.
