The Capital Hospice Center of Excellence Program is a partnership between Capital Hospice and select nursing homes that wish to improve access to quality end-of-life care. They do so by offering residents and their families the comprehensive, specialized services of Capital Hospice in addition to the facility’s expertise in providing long term care.
By becoming a Capital Hospice Center of Excellence, the nursing home has chosen to provide a comprehensive end-of-life program to their residents and families, utilizing Capital Hospice’s expertise in spiritual, emotional, and clinical management. Facility staff also receive education and support from Capital Hospice.
Some of the individual components that distinguish a Center of Excellence include:
Capital Hospice provides residents with specialized physical, emotional and spiritual support earlier in the course of their illness. This affords the resident an opportunity to derive the most benefit from specialized hospice and palliative services from the caring experts – nurses, social workers, chaplains, and others – on the Capital Hospice team.
Capital Hospice professional counselors with advanced training in end-of-life care assist family members in addressing emotional issues that often arise while caring for a loved one. The importance of attending to the caregiver’s emotional needs is often equal to that of the care given to the patient.
Capital Hospice physicians are board-certified experts in end-of-life care. They assist families in determining goals of care, and make recommendations to improve comfort and quality of life of the patient. They have special expertise in controlling pain and other symptoms, and can offer suggestions to the patient’s primary physician regarding treatment options.
Capital Hospice provides regular, advanced training programs for the nursing home or long term care facility staff to better help them identify and manage pain and symptom issues to maximize the resident’s comfort.
