Home Hospice Care

The hallmark of hospice care is that it is delivered to you at home, where you can be in familiar and comfortable surroundings, among family and loved ones. Your individualized plan of care is coordinated by your hospice nurse as part of a team that includes the doctor, along with a hospice social worker, chaplain, certified nursing assistant, and specially-trained volunteers. Additional resources, such as a dietician, physical therapist, occupational therapist or use of art therapy for example, are available as needed.

Grieving family and loved ones will be followed by our bereavement staff for 13 months, and may choose individual or group support and counseling.

Our service area includes the counties of Arlington, Fairfax, Fauquier, Loudoun, Prince William and Stafford and the metropolitan areas of Alexandria, Arlington, Falls Church, Fairfax, Manassas, Manassas Park and Warrenton in Virginia; Washington DC and Prince George’s County, Maryland.
