Make your wishes known now, save anguish later

Recent news stories have raised public awareness about why you should document in advance your wishes regarding medical care should there come a time when you are unable to speak for yourself. By using the Advance Directive and Living Will form you can go into detail about what measures you want and don't want. And doing so now can save your loved ones tremendous anguish later.

We recommend:

Discuss your wishes with your loved ones
Fill out the forms
Have the forms witnessed and signed by two people

Although you don't need an attorney to fill out or execute the forms, you might consider giving your lawyer a completed copy.

Make sure you have copies of the completed, signed and witnessed forms for your loved ones, your doctor, and a copy at home that can be easily accessed in an emergency.
